Clinical Wellness - Tailored Nursing Support
Delivered by Provocatus
Clinical Wellness is one of the Veteran Access Scheme’s (VAS) Pillars of Care. This means that Provocatus can provide tailored nursing support to anyone who has served or is soon to transition out of the Australian Defence Force (ADF). It is available to all veterans, regardless of their DVA status or accepted conditions.
Through a structured telehealth consult, our experienced Veteran Nurses assist participants to:
Understand and navigate key elements of the DVA system (e.g. RPBS entitlements, the RAP program, allied health, and mental health access)
Identify health and wellbeing concerns that may require new or additional support
Improve health literacy and confidence in managing their care within their existing local health team
Receive support in understanding and accessing available equipment, programs, and services
Where clinically appropriate, our nurses may provide ongoing assistance, help coordinate referrals, or liaise with local GPs and clinics to support broader care needs.
This program does not replace a veteran’s usual care team. Rather, it enhances their ability to engage with local GPs, allied health providers, and community services, particularly for those who may not currently be accessing appropriate care or are unaware of their entitlements.
Veterans can self-refer, or preferably be referred by their GP or another health provider.
